LIFESPAN DIRECTORY

Many of you have requested a Lifespan Directory so you can stay in touch and gather when Lifespan Academy is no longer meeting.  If you wish to be included in the directory, please complete this form.

We are not requiring you to complete the address and phone number fields, but your name and email is required.  If you do not complete the address and phone number fields, they will not be included in the directory.

Directories will be available at the Lifespan Gathering Day on April 20th, 11 to 2, at the Lifespan Office at Trinity Presbyterian Church, 3003 Howell Mill Road, NW, Atlanta, GA 30327.
