INSTRUCTIONS ON THIS FORM:

• Begin by entering your contact information
• Select the classes you wish to attend.  Please indicate if you are attending in-person or virtually.  
• Registration is $69 for 8 weeks of classes.  That includes as many or few classes as you wish to take, in-person and virtual.
• You can pay with PayPal, credit card or check. 
• If you select PayPal, click or touch the PayPal button and you will be directed to PayPal.
• If you select credit card, enter your credit card information on our secure page and then click submit.  Your registration and payment will be processed when you click or touch submit.
• If you select check, you just click submit to send your registration. 
• If you selected to pay by check, confirm the total, make your check to Lifespan Resources, & mail to Lifespan, 3003 Howell Mill Rd NW, Atlanta, GA 30305.
• You will receive a confirmation email with all your selections and your method of payment.
