INSTRUCTIONS ON THIS FORM:

• Begin by entering your contact information
• Select the classes you wish to attend.  Please note there are three registration levels:  All access, In-person & Virtual.
• When you select your registration type and number of passes, you will see a subtotal.  The coupon for new participants will be taken off the registration subtotal.  If you are not a  new participant you will not see this item.
• Indicate if you wish to sign up for lunch and if you wish to make a donation to Lifespan.
• You will then see your total amount.
• You can pay with PayPal, credit card or check. 
• If you select PayPal, click or touch the PayPal button and you will be directed to PayPal.
• If you select credit card, enter your credit card information on our secure page and then click submit.  Your registration and payment will be processed when you click or touch submit.
• If you select check, you just click submit to send your registration. 
• You will receive a confirmation email with all your selections and your method of payment.  If you selected to pay by check, confirm the total, make your check to Lifespan Resources, & mail to Lifespan, 3003 Howell Mill Rd NW, Atlanta, GA 30305.
