INSTRUCTIONS ON THIS FORM:

• Begin by entering your contact information
• Select the classes you wish to attend.  We are asking that you make your selections again, even if you have already registered.  Now that we are virtual, there are no competing classes and you may choose differently.  
• Registration costs are:  $59 for virtual only and $84 for the POTENTIAL of virtual AND in-person.  We are hoping to gather for some limited in-person activities later in the session.  If you selected in-person only, or paid for lunches, we will contact you about your registration.  IF YOU HAVE ALREADY PAID, SKIP THIS SECTION AND SUBMIT YOUR REGISTRATION FOR CLASSES ONLY.
• You can pay with PayPal, credit card or check. 
• If you select PayPal, click or touch the PayPal button and you will be directed to PayPal.
• If you select credit card, enter your credit card information on our secure page and then click submit.  Your registration and payment will be processed when you click or touch submit.
• If you select check, you just click submit to send your registration. 
• You will receive a confirmation email with all your selections and your method of payment.  If you selected to pay by check, confirm the total, make your check to Lifespan Resources, & mail to Lifespan, 3003 Howell Mill Rd NW, Atlanta, GA 30305.
